Voting - Ballot Forms and Mailer

Attachments: return envelope. pdf; Registration and Absentee Ballot Request-Federal Post Card Application (FPCA).pdf

Dear Fellow U.S. Citizens:

The Federal Voting Assistance Program would prefer that you not use the 1995 version of the FPCA (SF-76), although all states will accept them if you have no choice but to use them.  Currently the U.S. Embassy on has available the 1995 version.

The FPCA was revised and improved in 2005 as follows:
- The new forms have space for an e-mail address (2.j.) to facilitate communication between local election officials and the voter.
- The new forms have boxes the voter can mark (4.c.) to indicate how he/she wants the blank ballot sent out from the state (as authorized by the state)
- The new forms also have far more space (6) for comments.
- The new form self-seals on three sides to better protect the voter's personal data from potential identity theft.

We encourage you to include an e-mail address and a piece of paper if you need additional space for comments.  Be sure seal all edges with tape.
The E-version of the FPCA

Voters may also use the electronic version of the FPCA (attached as a PDF). The FPCA may be mailed to local election officials through the international mail (with appropriate postage), or voters can print out the attached postage-paid mailing label, affix it to an envelope, and give it to your nearest U.S. Embassy or Consulate to mail in the FPCA in that envelope.  Both the electronic FPCA and postage-paid envelope template are available at www.fvap.gov and this website contains complete information on all your voting needs.

For ease of use the mail-in return envelope and Federal Pot Card Application form are included below attachments.
